The Health and Social Care Alliance Scotland (the ALLIANCE) is hosting Scotland’s fifth Self Management Awards during Self Management Week 2015, 28 September – 2 October.
The awards aim to highlight examples of good practice and to promote a range of innovative self management work from all over Scotland.
Individuals, voluntary organisations, health and social care organisations and practitioners are encouraged to submit nominations to a range of categories.
The theme of Self Management Week 2015 is Inspiration – Who or what inspires you to self manage? How do you inspire others to self manage? Do you know a truly inspirational person or organisation?
Individuals, projects or organisations can submit entries to any or all of the following categories:
- Self Management Supporting Health Board of the Year
- Inspirational Person of the Year
- Self Management Partnership of the Year
- Self Management Project of the Year
- Self Management Resource of the Year
